Key Responsibilities- Administer routine services in caring for subacute, convalescent, and chronic residents of the Nursing Home.
- Participate in planning, implementation, and evaluation of nursing care.
- Direct and supervise Certified Nursing Assistants (CNAs) assigned to your wing.
- Maintain continuity of resident care during respective tours of duty and prioritize nursing tasks.
- Assign residents to CNAs, supervise, and assist nursing staff in planning care for each resident.
- Assist with orientation of new personnel and make frequent rounds on residents.
- Observe and record symptoms and/or reactions to treatments and changes in condition, and inform physicians of changes in resident's condition.
- Maintain good relations with visitors, families, and other people while on duty.
- Work closely with supervisors in dealing with personnel, residents, and resident's family situations.
- Delegate authority to other licensed persons as necessary.
- Report to relief personnel all pertinent information for continuity of care and assure follow-through care as needed.
- Start IVs, administer IV fluids and IV medications, insert, irrigate, and feed residents through nasogastric and gastrostomy tubes, administer aerosol treatments as ordered, and perform all facets of admission and discharge of residents.
- Administer medications to residents (orally, IM, and SubQ, topically, and intradermally), order medications for use on the unit as needed, and maintain level of stock drugs, equipment, and supply needs.
- Adhere to safety and infection control standards, and policies and procedures, and demonstrate use of Standard Precautions in all aspects of nursing care.
- Supervise private sitters assigned to residents on units, maintain and perform sterile and clean dressings, and monitor wounds, including depth, circumference, edge approximating, and tissue granulation formation.
- Chart appropriately and do dressing changes as ordered by physicians, compare present resident care with documented plan of care, and complete a monthly summary on assigned residents using this information.
- Order treatment (such as laboratory tests, x-rays, or diets, etc.) online, and in the absence of a Nursing Supervisor and/or DON, assure adequate staffing by covering for any call-offs.
- Assure that two licensed persons sign off all physician's orders (on the physician's order sheet and on the MAR) and that a 990 form is completed for all verbal or telephone orders, transcribe physician orders, and assist in maintaining a safe environment for all residents.
- Report to appropriate managerial staff equipment that is out of order, suspicion of resident abuse, mistreatment, neglect, and reporting of suspicion of criminal activities.
- Demonstrate age-specific competencies, follow policies, procedures, and safety standards, complete required education assignments annually, work toward achieving goals and objectives, and participate in quality improvement initiatives as requested.
- Current TN LPN license required.
- CPR certification required within 1 year.
- One (1) year experience as an LPN.
