Job Title: Licensed Practical Nurse (LPN)Location: North Wales, PA

IntelyCare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in North Wales, PA. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities, providing high-quality patient care and making a meaningful difference in the lives of our residents.

Job Responsibilities:
  • Provide compassionate nursing care to residents in post-acute facilities
  • Monitor residents' physical and mental conditions, tracking vital signs and documenting medical histories
  • Administer medications, track their use and effects, and maintain accurate medical records
  • Cultivate a comfortable and safe environment for residents
  • Assist with the setup of IVs, immunizations, and phlebotomy procedures
  • Keep track of medical supplies and maintain a proper inventory
  • Follow and adhere to resident care plans
  • Work with staff to orient and train new colleagues
Qualifications and Skills:
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
