Gentiva Hospice is seeking a skilled Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Responsibilities- Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Provide physical assessments and complete documentation within regulatory time frames.
- Make recommendations to the Interdisciplinary Group (IDG) based on F2F assessment findings.
- Notify the hospice agency when unable to complete a scheduled F2F visit.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
- Competitive salary range: $60.00/hour - $65.00/hour.
