RN Case Manager

Infinity Hospice Care is seeking a skilled RN Case Manager to coordinate and provide high-quality hospice care to patients and families. The ideal candidate will have experience in nursing, preferably in an acute care setting or home health, hospice, or a comparable field.
Responsibilities:
- Coordinate patient care from an Interdisciplinary Group approach.
- Provide hands-on clinical nursing, interacting with patients, family, physicians, and other healthcare professionals.
- Assess and implement a pain/symptom management program.
- Facilitate freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues.
- Provide support and education for families based on the hospice philosophy of care.
- Participate in Interdisciplinary Group meetings and update the Plan of Care for assigned patients.
- Ensure all services provided are in accordance with the patient's Plan of Care.
Qualifications:
- An active license as a Registered Nurse in good standing in the state where nursing services will be performed.
- Graduate from an accredited school of nursing and at least 1 to 2 years of experience in nursing in an acute care setting or 1 year of experience in home health, hospice, or comparable experience.
- Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care.
On Call: The RN Case Manager will be on call according to a prepared schedule. During on-call hours, the RN Case Manager is expected to follow the On Call Protocol and respond to all pages within 20 minutes.
