You will work closely with our interdisciplinary team to develop and implement care plans that meet the unique needs of each resident. Job Responsibilities
* Develop and implement individualized care plans for residents
* Administer medications as delegated by a licensed nurse
* Assist residents with daily living activities, including bathing, toileting, grooming, and dressing/undressing
* Provide psycho-social support and personal care to assigned residents
* Assist residents with meals and snacks, and provide water and supplements as appropriate
* Document resident care and medication administration
* Communicate changing conditions to the charge nurse
* Participate in departmental committees and task forces as needed

Requirements
* High school graduate or GED preferred
* Minimum age of 18 preferred
* Prior clinical or nursing assistant experience in long-term care preferred
* Certification as a nursing assistant and completion of a state-approved Trained Medication Assistant course
* Ability to maintain all department-specific required certifications and complete yearly competency testing
