Key Responsibilities
  • Lead a multidisciplinary team in developing, refining, and communicating care plans for a defined patient population.
  • Implement clinical pathways, addressing the educational needs of patients and families while monitoring for any deviations from the established plan.
  • Partner with community liaisons to effectively manage care transitions, ensuring seamless communication between care settings and ongoing providers.
  • Collaborate with Social Work, Utilization Review, Clinical Documentation Specialists, and other necessary personnel to optimize financial and care resources for patients and their families.
  • Coordinate care with the healthcare team to enhance patient throughput and reduce length of stay.
  • Communicate proficiently with healthcare providers, patients, families, and visitors, demonstrating effective interaction with individuals across all age groups.
  • Ensure that discharge instructions related to care coordination are documented and comprehended by patients and their families or caregivers.
  • Work closely with physicians, unit managers, unit staff, and interdisciplinary teams to facilitate safe and timely discharges.
Qualifications
  • Graduate of an accredited nursing program.
  • Current Texas (or NLC Nursing Licensure Compact) nursing license.
  • Preferred experience in Care Coordination/Case Management as an RN within an acute care setting.
  • Proficient in reading, writing, and communicating in English.
