Job Summary



Premise Health Systems is seeking a seasoned Licensed Practical Nurse (LPN) to join our team at our Health and Wellness Center in Austin, MN. As a key member of our healthcare team, you will provide high-quality patient care and support to our clients.



Key Responsibilities:





  • Conduct patient interviews to determine medical problems and document in charts for physician review

  • Obtain and record patient vital signs and weight

  • Assist physicians with patient examinations as needed

  • Review physician orders, lab requests, and follow-up needs with patients

  • Document services performed on appropriate forms

  • Dress wounds as instructed, apply compresses, ice, and heat packs

  • Report adverse reactions to medication or treatment to medical personnel in charge

  • Administer specified medication orally or by subcutaneous or intermuscular injection and note time and amount on patient charts

  • Assemble and use clinic equipment

  • Collect samples, such as urine, blood, and sputum, from patients for testing and perform routine laboratory tests on samples

  • Sterilize equipment and supplies

  • Clean rooms and change linens

  • Maintain supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies

  • Respond to patient or client telephone inquiries by evaluating and responding to patient/client problems and questions

  • Follow up with patients who miss appointments

  • Follow up with patients on lab results, including scheduling of additional tests

  • Coordinate referrals for patients through insurance and other physician offices



Requirements:



To be successful in this role, you will need:





  • Graduate of a National League for Nursing-accredited program, Associate's degree (A.S.) and/or equivalent from a two-year college or technical school

  • Licensed as a Practical or Vocational Nurse

  • Current hands-on certification in Basic Life Support; Advanced Cardiac Life Support may also be required based on scope of contract services

  • At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting
