Job Summary

We are seeking a highly skilled and experienced Patient Care Assessment Specialist to join our team at Regency Integrated Health Services.


Key Responsibilities

The successful candidate will be responsible for the coordination of the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payer program requirements.


Key duties will include:



  • Coordinating the RAI process to ensure accurate and timely completion of resident assessments
  • Ensuring assessments accurately reflect the physical, mental, and psychosocial status of each resident
  • Communicating effectively with the interdisciplinary team
  • FOLLOWING all Regency Integrated Health Services policies and procedures

Requirements

To be successful in this role, you will need:



  • A graduate degree in nursing from an accredited school
  • Current licensure in the state where employment occurs
  • A minimum of two years of healthcare experience, preferably in a related field
  • Experience with MDS completion, reimbursement, clinical resource utilization, and/or case management is highly desirable
