The OpportunityWe are seeking a highly skilled and compassionate Nurse Practitioner or Physician Assistant to join our team in Arizona. As a Primary Care Specialist, you will play a critical role in delivering exceptional care to our geriatric patient population.
Key Responsibilities- Provide comprehensive primary care in an outpatient setting, focusing on preventive care, chronic disease management, and mental health support.
- Partner with our care team to leverage our proprietary Electronic Health Record and drive behavior change in our patients.
- Collaborate with our medical director to develop and implement innovative care models that improve patient outcomes.
- Participate in our value-based care environment, where you will have the opportunity to lead systemic change in healthcare delivery.
- At least 2 years of primary care experience, with a focus on geriatric care.
- Current licensure in the state of Arizona, with the ability to obtain licensure if necessary.
- Excellent communication and interpersonal skills, with the ability to build successful relationships with team members and patients.
- Board Certification in a relevant specialty.
- Spanish language fluency a plus.
