TotalMed is seeking a highly skilled Clinical Care Coordinator to join our team of healthcare professionals in providing exceptional patient care in acute settings. About the Role

The Clinical Care Coordinator will be responsible for assessing, planning, and implementing patient care plans in collaboration with the healthcare team. The ideal candidate will have strong communication and adaptability skills, as well as experience working in Telemetry units. A current RN license and BLS certifications are required for this position.

Responsibilities
  • Monitor patients' cardiac rhythms and vital signs using telemetry equipment.
  • Assess patients' conditions, administer medications, and provide interventions as necessary.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Educate patients and families on cardiac conditions, treatments, and self-care measures.
Requirements
  • Current RN license
  • BLS Certifications
  • Two years of recent Telemetry experience
  • Strong communication and adaptability skills

TotalMed offers a competitive compensation package, including a salary range of $85,000 - $110,000 per year, based on experience and qualifications.
