Job Summary

Integral Care is seeking a qualified Case Manager to provide services to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions. The ideal candidate will utilize a recovery based and trauma-informed perspective to develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with diverse community members.

Key Responsibilities
  • Provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.
  • Work with individuals in developing skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.
  • Facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities as needed.
Requirements
  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Current and valid driver's license and a clean driving record.
