The Nurse Practitioner (NP) plays a crucial role in delivering direct patient care within ChenMed's dedicated medical centers. Key responsibilities include:

  • Conducting comprehensive geriatric assessments, including medical history and physical examinations.
  • Diagnosing and treating various health conditions while developing tailored nursing care plans.
  • Providing health education and facilitating referrals to physicians and case management services.
  • Ensuring meticulous documentation in line with ChenMed's quality and service standards.

The Nurse Practitioner must exhibit the capability to work autonomously while also collaborating effectively with other healthcare professionals. Regular consultations with management and medical directors are essential to maintain compliance with established guidelines.

Core Responsibilities:

  • Independently assess and address both acute and chronic clinical issues.
  • Document physical assessments and patient histories, analyzing trends to formulate management plans.
  • Plan patient care with a deep understanding of the specific population served, considering cultural backgrounds and support systems.
  • Manage patient care through:
    • Writing orders for admissions, transfers, and discharges.
    • Ordering and interpreting laboratory and diagnostic tests.
    • Prescribing appropriate medications and treatments.
    • Referring patients to specialists as necessary.
  • Participate in clinical rounds and communicate strategies with the patient care team.
  • Recognize and respond to situations requiring immediate medical attention.
  • Utilize advanced communication skills to enhance patient care processes.

Additional Responsibilities:

  • Collaborate with multidisciplinary teams to evaluate and improve healthcare delivery systems.
  • Engage in professional development activities to enhance personal and professional growth.
  • Work closely with healthcare providers to ensure smooth transitions for patients to lower levels of care.
  • Facilitate patient and family conferences to discuss treatment goals and optimize resource utilization.
  • Support families in making informed decisions regarding care options.
  • Facilitate appropriate discharge planning and utilize preferred providers for additional services.
  • Prescribe medications in accordance with state regulations.
  • Perform other duties as assigned by management.

Required Knowledge, Skills, and Abilities:

  • Proven track record of achieving clinical performance metrics.
  • Strong critical thinking and problem-solving abilities.
  • Excellent communication and interpersonal skills.
  • Effective time management skills and the ability to thrive under pressure.
  • Empathy and strong listening skills.

Education and Experience Requirements:

  • Advanced degree in Nursing (ARNP or equivalent) is required.
  • Current Nurse Practitioner Certification in the relevant state is mandatory.
  • Board certification by AANP or ANCC is preferred.
  • Current DEA registration for controlled substances may be required.
  • A minimum of one year of clinical experience is preferred, though strong new graduates will be considered.
  • Basic Life Support (BLS) certification is required within the first 90 days of employment.
