The Nurse Practitioner (NP) is an integral member of our clinical operations team, responsible for delivering direct patient care within ChenMed/Dedicated medical centers. Key responsibilities encompass geriatric evaluations, comprehensive medical histories, physical examinations, diagnoses, treatment plans, health education, and appropriate referrals. This role also involves meticulous documentation in line with ChenMed's standards for quality, service, productivity, and teamwork. Participation in clinical rounds and conferences, along with thorough documentation through written progress notes and summaries, is essential.
The Nurse Practitioner must exhibit the capability to operate autonomously while collaborating with other healthcare professionals. Regular consultations with management, physicians, and medical directors are necessary to ensure adherence to established guidelines. Participation in risk management and quality assurance programs, as well as clinical meetings, may be required.
KEY RESPONSIBILITIES:
- Conduct thorough assessments of acute and non-acute clinical issues. Document physical assessments and patient histories, analyze patient condition trends, and formulate a comprehensive patient management plan based on gathered data.
- Develop patient care strategies rooted in a deep understanding of the specific patient demographic, anticipating physiological and psychological challenges while considering cultural backgrounds and support systems.
- Manage patient care, which includes:
- Writing orders for admissions, transfers, and discharges.
- Ordering and interpreting necessary laboratory and diagnostic tests.
- Prescribing appropriate medications and treatments.
- Referring patients for specialized consultations as needed.
- Maintaining detailed documentation through progress notes and summaries.
- Performing invasive procedures independently upon achieving documented competency.
- Engaging in patient care rounds and conferences, communicating management strategies to the care team.
- Identifying urgent situations requiring immediate physician intervention and initiating life-saving measures when necessary.
- Utilizing advanced communication skills to resolve complex issues and enhance patient service.
- Collaborate with multidisciplinary team members to assess and refine healthcare delivery systems, implementing new practices as appropriate.
- Participate in professional development activities that foster personal and professional growth.
- Work in tandem with physicians, nurses, and other healthcare professionals to facilitate patient transitions to lower levels of care when medically appropriate.
- Introduce yourself to patients and families, clarifying the role of the nurse practitioner.
- Lead patient and family conferences to discuss treatment objectives, optimize resource use, and provide education on post-care needs.
- Foster collaborative relationships to empower patients and families in making informed decisions regarding care goals.
- Ensure appropriate discharge planning and utilize preferred providers for additional services as necessary.
- Prescribe medications in accordance with state regulations.
- Perform other duties as assigned by management.

- Basic computer proficiency and familiarity with Microsoft Office Suite.
- This position may necessitate local travel.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Advanced degree in Nursing (ARNP or equivalent) is required.
- Current Nurse Practitioner Certification in the applicable state is mandatory.
- Board certification by AANP or ANCC is preferred, though not mandatory in all states.
- A current DEA number for controlled substances may be required based on state regulations.
- A minimum of one year of clinical experience is preferred, though strong new graduates will be considered.
- Basic Life Support (BLS) certification from an accredited organization is required within the first 90 days of employment.
