PruittHealth Hospice is seeking a dedicated Licensed Practical Nurse (LPN) to join our specialized team focused on providing exceptional hospice care. Your role will be crucial in delivering compassionate nursing services to patients in their homes or care facilities, ensuring their comfort and dignity during this critical time.
Key Responsibilities:
- Deliver palliative nursing care as per the established treatment and care plans.
- Collaborate effectively with the interdisciplinary team, including patients, caregivers, and physicians.
- Implement pain and symptom management strategies to enhance patient comfort.
- Address the holistic needs of terminally ill patients and their families, providing emotional and social support.
- Maintain compliance with all agency policies regarding documentation and patient care.
- Educate patients and families about care provisions and available services.
- Participate in team meetings and contribute to the development of patient care plans.
- Engage in ongoing professional development and performance improvement initiatives.
Qualifications:
Applicants must possess a current and unrestricted LPN license in the state of practice. A thorough understanding of hospice regulations and excellent communication skills are essential.
