Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Fort Collins, Colorado.

Job Summary:

We are seeking a dedicated and experienced Physical Therapist to work with our multidisciplinary team, providing high-quality care to clients in independent living and assisted living communities.

As a Home Health Physical Therapist, you will be responsible for conducting thorough assessments, developing and implementing individualized care plans, and providing ongoing therapy services to clients.

Responsibilities:
  • Conduct thorough assessments of clients to identify their physical therapy needs and develop personalized care plans.
  • Provide high-quality physical therapy services, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Collaborate with the multidisciplinary team to ensure comprehensive care and make referrals as necessary.
  • Communicate effectively with clients, family members, and community staff to ensure seamless care transitions.
  • Document all interactions, assessments, and interventions in a timely and accurate manner.
Requirements:
  • Current CO Physical Therapist license.
  • Outpatient experience preferred but not required.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Dynamic and strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
