Optimal Home Care is seeking a licensed Physical Therapist to provide high-quality patient care in the comfort of their own homes. As a Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Assess patients and develop personalized treatment plans to improve their quality of life
  • Provide evidence-based physical therapy services to patients in their homes
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Graduate of a physical therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE)
  • Current licensure in the state of Colorado
  • At least one year of experience as a Physical Therapist
  • Excellent communication and interpersonal skills
