About the Role

We are seeking a skilled Speech Therapist to join our team at Heartland Home Health. As a Speech Therapist, you will play a vital role in providing high-quality patient care and support to individuals with speech and language disorders.

Key Responsibilities
  • Provide speech and language therapy services to patients in their homes, under the direction of the attending physician.
  • Develop and implement individualized treatment plans to address patients' speech and language needs.
  • Collaborate with other healthcare professionals, including nurses and family members, to ensure comprehensive patient care.
  • Conduct assessments and evaluations to identify patients' speech and language strengths and weaknesses.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Master's degree in Speech Therapy from an accredited institution.
  • Licensed and in good standing in the state where you will practice.
  • Current CPR Certification.
  • Minimum of two years of experience as a Speech Therapist in a multidisciplinary setting.
  • Knowledge of speech therapy principles, concepts, and methodologies appropriate for terminally ill patients.
  • Excellent communication and interpersonal skills.
  • Ability to develop and maintain rapport with patients and families.
  • Ability to travel 100% daily.
