Job Summary

We are seeking a skilled and motivated Licensed Practical Nurse (LPN) to join our healthcare team at an SNF (Skilled Nursing Facility) in Bellevue, WA.

Responsibilities
  • Understand and adhere to established policies and procedures.
  • Ensure adherence to Universal Precautions for resident and staff safety.
  • Prepare residents' medications for short-term absences following state regulations.
  • Collaborate with the healthcare team to implement the nursing care plan for each patient.
  • Initiate preventive and rehabilitative nursing procedures as appropriate.
  • Observe signs and symptoms and report to the physician and RN, including reactions to treatments and changes in the patient's physical or emotional condition.
  • Teach and counsel patients and their families/significant others on nursing care needs and related issues for home care.
  • Participate in medical record audits as assigned.
  • Attend staff meetings, patient care conferences, and in-services as scheduled.
  • Read Daily Communication Logs for resident information.
  • Document any changes in resident status in Progress Notes and review them regularly.
  • Complete resident treatments as ordered and scheduled.
  • Assist with personal care and physical support as needed.
Requirements
  • Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state.
  • Active BLS (Basic Life Support) certification.
  • Preferred minimum of one (1) year of nursing experience.
  • Strong communication and interpersonal skills.
  • Ability to work well in a fast-paced SNF environment.
  • Two (2) positive work references
  • Successfully complete and pass a background check and drug test.
