Key Responsibilities:- Manage triage calls and messages for the clinic
- Work closely with Providers to ensure seamless patient care
- Develop and implement effective patient care plans

- Diploma in Practical Nursing (LPN) or Associate of Applied Science in Registered Nursing (RN)
- Active LPN or RN license
- 2-3 years of experience in primary care nursing
