About the Role

We are seeking a skilled and compassionate LPN to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality nursing services to patients in their homes, promoting independence, dignity, and comfort.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and national standards of care.
  • Develop and implement individualized care plans, focusing on patient-centered goals and outcomes.
  • Conduct thorough assessments, identify patient needs, and prioritize interventions to ensure optimal care.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care.
  • Communicate effectively with patients, families, and caregivers, ensuring seamless transitions and continuity of care.
Requirements
  • Graduate of an accredited LPN program or equivalent.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
