As a Comprehensive Primary Care Specialist, you will provide comprehensive primary care services to patients in the comfort of their own homes. Responsibilities:

Conduct in-home assessments and evaluations of patients' health status, taking into account their medical history, current condition, and individual needs.

Develop personalized care plans based on patients' unique requirements, ensuring that their care is tailored to their specific needs.

Perform physical examinations, order diagnostic tests, and interpret results, making informed decisions to optimize patient care.

Prescribe medications and treatments as appropriate, always prioritizing patient safety and well-being.

Educate patients and their families about preventive care, medication management, and lifestyle modifications, empowering them to take an active role in their health.

Coordinate care with other healthcare providers and community resources to ensure continuity of care, ensuring that patients receive seamless and comprehensive care.

Document all patient encounters accurately and in a timely manner, maintaining up-to-date and detailed records.

Participate in interdisciplinary team meetings and case conferences, sharing knowledge and best practices to improve patient outcomes.

Maintain licensure and certification requirements, including continuing education, to stay up-to-date with the latest developments in primary care.

Adhere to organizational policies, procedures, and standards of practice, upholding the highest standards of care and professionalism.

Qualifications:

A Master's or Doctorate degree in Nursing from an accredited program is required, as is current licensure as a Nurse Practitioner in the state of Missouri.

Certification as a Nurse Practitioner by a nationally recognized certifying body is also required, as is a minimum of 2 years of experience as a Nurse Practitioner, preferably in primary care or home health.

Strong clinical assessment and diagnostic skills are essential, as is excellent communication and interpersonal skills, enabling you to work effectively with patients, families, and healthcare teams.

Ability to work independently and as part of a collaborative team is also required, as is proficiency in electronic health record (EHR) documentation.

A valid driver's license and reliable transportation are necessary, as you will need to travel to patients' homes to provide care.

Salary Estimate:

The estimated salary for this position is around $120,000 per year, based on national averages and industry standards.
