About the RoleTidewell Hospice, the sole provider of hospice services in four counties (Manatee, Sarasota, Charlotte & DeSoto) of sunny Southwest Florida, is seeking an experienced and team-oriented RN Triage Case Manager.
Key Responsibilities- Reviews and updates the care plan, physician orders, pharmacy profile and other records.
- Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.
- Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
- BSN or MSN preferred.
- Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
