LPN MDS Nurse Job Summary

We are seeking a skilled LPN MDS Nurse to join our team at Lake Pointe Healthcare Center, a member of the CommuniCare Family of Companies. As a key member of our Operations team, you will play a crucial role in ensuring the accuracy and timeliness of MDS assessments and coordination of the RAI process.

Responsibilities
  • Accurately complete MDS assessments and coordinate the RAI process
  • Provide Medicare, Medicaid, and managed care oversight to ensure appropriate clinical services are provided and reimbursement is received
  • Ensure compliance with federal and state regulations, as well as CommuniCare Family of Companies' guidelines and policies
Requirements
  • Graduate of an accredited school of nursing with LPN licensure
  • Three years of experience in a long-term care environment, with experience in MDS/RAI process and/or case management preferred
