Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to serve as our Director of Intake and Patient Services. This key leadership role will work closely with our business development team and referral sources to direct the day-to-day operations of our Intake Coordination team.

Key Responsibilities
  • Direct the day-to-day operations of the Intake Coordination team
  • Collaborate with the business development team and referral sources to drive growth and improve patient outcomes
  • Develop and implement strategies to increase staff-to-patient ratios and improve clinical visit frequency
  • Lead by example, demonstrating exceptional leadership skills and a commitment to delivering high-quality patient care
  • Recruit, train, and develop a high-performing team of Intake Coordinators
  • Monitor and evaluate team performance, identifying areas for improvement and implementing corrective actions as needed
Requirements
  • Current RN licensure (in good standing) in the state(s) of practice
  • At least 1 year of hospice experience and 2 years of nursing experience
  • Valid driver's license and reliable transportation
  • Excellent leadership and communication skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
  • Strong analytical and problem-solving skills
