Job Summary

Sea Mar Community Health Centers is seeking a skilled Healthcare Integration Specialist to join our Care Management department in Anacortes, WA. As a key member of our interdisciplinary team, you will provide Health Home services and support to patients with complex medical, behavioral health, and social concerns.

The ideal candidate will have knowledge of Community Outreach or Case Management and be able to work effectively with patients, families, and healthcare providers to develop and implement personalized care plans. Strong communication and interpersonal skills are essential for success in this role.

Key Responsibilities:

  • Provide Health Home services and support to patients with complex medical, behavioral health, and social concerns.
  • Develop and implement personalized care plans in collaboration with patients, families, and healthcare providers.
  • Conduct timely and effective screenings and referrals to internal Sea Mar service providers, community-based resources, and emergency services when indicated.
  • Work with patients to create a Health Action Plan, including long and short-term goals with actionable steps to manage chronic health conditions.
  • Follow up with patients regularly to evaluate progress made towards completing their Health Action Plan goals.

Requirements:

  • Bachelor's degree in Human Services, Health Sciences, or related field with experience in social service case management or care coordination.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong communication and interpersonal skills.
  • Knowledge of Community Resources and experience working with underserved populations preferred.
