Job Summary

We are looking for a dedicated and compassionate Physical Therapist to provide personalized rehabilitation therapy to patients in their homes and care facilities. As a member of our interdisciplinary care team, you will conduct comprehensive assessments, develop treatment plans, and deliver high-quality care to patients with a range of needs.

Responsibilities
  • Practice independently in the community as part of an interdisciplinary care team.
  • Conduct comprehensive assessments of patients, determining their needs and developing courses of action, including treatment plans and outcomes.
  • Deliver personalized rehabilitation therapy to patients in their homes or care facilities.
  • Constantly evaluate evolving patient needs and respond with plan of care adjustments.
Requirements
  • Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside the borough of Manhattan.

  • Competitive pay rate: $70-$75 per visit for all areas other than Bronx, and $90-$95 per visit for the Bronx.
