Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to ensure proper follow-through and documentation of resident care.
- Assist in conducting resident evaluations and gathering data to inform care decisions.
- Monitor and respond to resident incidents, documenting and reporting as required.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Graduate of an approved college or school of nursing.
- Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Minimum of one year experience in long-term care, assisted living, or home health.
- Strong assessment and communication skills, with the ability to work effectively in a team environment.
