We are seeking a highly skilled and experienced Home Care Network Leader to join our team at Northwell Health. As a key member of our leadership team, you will be responsible for overseeing the daily operations of our Home Care Network, ensuring the delivery of high-quality patient care and services.
Key Responsibilities- Leadership and Management
- Supervise and evaluate the performance of nurse liaisons, providing guidance and support to ensure the delivery of excellent patient care.
- Collaborate with management to develop and implement policies and procedures that promote the growth and development of the Home Care Network.
- Lead the activities of assigned team members, promoting a culture of teamwork and collaboration.
- Quality and Compliance
- Ensure that all patient care services are delivered in accordance with regulatory guidelines and standards of quality.
- Monitor and analyze data to identify areas for improvement and implement strategies to enhance patient outcomes.
- Collaborate with staff to develop and implement quality improvement initiatives.
- Communication and Collaboration
- Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
- Collaborate with management and staff to develop and implement strategies to promote the growth and development of the Home Care Network.
- Participate in professional committees and working groups to advance the mission and vision of Northwell Health.
- Education
- Graduate from an accredited School of Nursing required.
- Experience
- 4-6 years of relevant experience and 0-2 years of leadership/management experience, required.
- Certifications
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
The salary range for this position is $91,400-$158,100/year. Northwell Health offers a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 401(k) retirement plan and paid time off.
