Key Responsibilities
  • Leadership and Management
    • Supervise and evaluate the performance of nurse liaisons, providing guidance and support to ensure the delivery of excellent patient care.
    • Collaborate with management to develop and implement policies and procedures that promote the growth and development of the Home Care Network.
    • Lead the activities of assigned team members, promoting a culture of teamwork and collaboration.
  • Quality and Compliance
    • Ensure that all patient care services are delivered in accordance with regulatory guidelines and standards of quality.
    • Monitor and analyze data to identify areas for improvement and implement strategies to enhance patient outcomes.
    • Collaborate with staff to develop and implement quality improvement initiatives.
  • Communication and Collaboration
    • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
    • Collaborate with management and staff to develop and implement strategies to promote the growth and development of the Home Care Network.
    • Participate in professional committees and working groups to advance the mission and vision of Northwell Health.
Requirements
  • Education
    • Graduate from an accredited School of Nursing required.
  • Experience
    • 4-6 years of relevant experience and 0-2 years of leadership/management experience, required.
  • Certifications
    • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
Salary and Benefits

The salary range for this position is $91,400-$158,100/year. Northwell Health offers a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 401(k) retirement plan and paid time off.
