Licensed Practical Nurse/LPN Full Time

As a Licensed Practical Nurse (LPN) at Roosevelt Care Centers, you will provide high-quality nursing care to our residents in a long-term care facility. In the absence of a Registered Nurse (RN), you will act as a Team Leader in your unit, ensuring the delivery of exceptional care and support to our residents and their families.
Key Responsibilities:
- Provide direct care to assigned residents, including administration of medications and treatments, and complete rounds as necessary.
- Monitor residents' conditions and respond appropriately, notifying the RN or Nursing Supervisor as needed.
- Document pertinent information regarding resident responses and nursing interventions in their medical records.
- Assist in the completion of Minimum Data Set (MDS) and care planning process under RN direction.
- Orient residents and their families to our long-term care environment and maintain liaison with families.
- Participate in committees and conferences, and orient and assist other clinical personnel.
- Initiate appropriate referrals to other departments, services, and/or outside agencies for resident care.
- Provide comprehensive patient/resident care and consult with supervisors and others regarding significant changes in resident's condition.
- Ensure that medical and emergency equipment/devices, including alarms, are in operating condition and available and functioning.
- Participate in wound care rounds and assist in the orientation and training of new personnel.
Additional Responsibilities:
Maintain established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards; promote adherence to Resident Bill of Rights.
Requirements:
Ability to read, write, speak, and understand English sufficiently to perform the job. Graduation from an accredited school of practical nursing plus a current NJ license to practice as a Licensed Practical Nurse. One year of experience in clinical nursing is preferred.
Work Schedule:
Shifts: 7am-3pm, 3pm-11pm, 11pm-7am
