Collegial Multispecialty Group PracticeOur multispecialty group practice offers a collaborative and supportive environment, with a large referral base and a focus on team-based care. You'll work alongside talented peers to provide comprehensive care to our patients, with excellent support staff and dedicated practice management systems.

Primary Responsibilities- Perform comprehensive and problem-focused medical history and physical examinations
- Develop and implement treatment plans for patients assigned within the primary care team
- Develop and actualize plans for the primary care team as assigned
- Provide guidance to nursing personnel towards treatment plans and care issues
- Document treatment plans in the medical record
- Collaborate with physician and documents physician guidance as needed
- Coordinate care for patients

Optum NY/NJ was formed in 2022 by bringing together Riverside Medical Group, CareMount Medical and ProHealth Care. Our regional alignment combines resources and services across the care continuum, from preventative medicine to diagnostics to treatment and beyond. As a Patient-Centered Medical Home, we provide patient-focused medical care to the entire family.
Required Qualifications- Advanced Nurse Practitioner Certification in Primary Care, Family Medicine, Geriatric, Adult or Physician Assistant certification
- Active unrestricted APRN or PA-C license in New York, or ability to obtain prior to start
- Active and unrestricted DEA License or ability to obtain prior to start
New York Residents Only: The salary range for New York residents is $109,513 to $147,146 per year. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
