We are seeking a talented Full Time RN Case Manager to join our team at Hospice of Central Pennsylvania. As a RN Case Manager, you will be responsible for providing nursing care in accordance with each patient's plan of care within our service area.
Key Responsibilities- Evaluate, establish and maintain a comprehensive plan of care that is individualized to specific patient circumstances.
- Provide direct clinical care and coordinate the care offered from all germane team members.
- Assess a patient's physical, psychosocial and spiritual needs with each visit.
- Establish and maintain an individualized and up-to-date plan of care, in conjunction with the interdisciplinary team and the primary physician.
- Facilitate and coordinate nursing and/or physician orders for the patient.
- Input new patient orders, as well as all patient documentation, into our software system in an accurate and timely manner.
- Order supplies and equipment as needed for patient care.
- Supervise Hospice Aides as per agency policy.
- Accept calls from patient/family/physician and relay information as appropriate.
- Participate in continuing education and agency training as directed.
- Recognize that quality improvement is a part of every staff member's job responsibilities.
- Identify and report areas for improvement and concerns for safety of patients, family members and staff.
- RN, with current PA license.
- Minimum of one year clinical nursing experience, with hospice or home care experience helpful but not mandatory.
- Excellent communication skills, including written, verbal and listening skills.
- Ability to track and complete multiple tasks at one time.
- Ability to work independently and as part of a team.
- Ability to work in a non-judgmental fashion with people of diverse backgrounds.
- Actively supports our philosophy of care and Key Performance Standards of Excellence.
- Ability to comprehend the stressors in one's own life and the stressors inherent in a job focused on the end of life.
- Proficient at basic computer/keyboard skills.
- Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
