Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team in a long-term care setting. As a LPN, you will be responsible for providing high-quality patient care, working collaboratively with the interdisciplinary team to develop and implement individualized care plans, and assuming leadership roles as needed.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
  • Develop and implement individualized care plans, taking into account patient needs, preferences, and goals
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication
  • Assume leadership roles as needed, including charge nurse responsibilities and participating in unit-based quality improvement initiatives
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Maryland
  • Basic computer skills, including proficiency in electronic medical records and other healthcare software
  • Ability to work effectively in a fast-paced, dynamic environment
  • Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members
