Key Responsibilities
  • Develop and implement individualized care plans in collaboration with the Registered Nurse and other healthcare professionals.
  • Provide education and training to individuals on health-related topics, including medication administration, wound care, and disease management.
  • Conduct regular assessments to identify potential health risks and develop strategies to mitigate them.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements
  • New York State Department of Education licensure as a Licensed Practical Nurse.
  • Valid NYS Driver License.
  • Long-term care experience and experience supporting individuals with intellectual and developmental disabilities preferred.
  • Flexibility to work varied shifts, including evenings and weekends.
  • Critical thinking skills and leadership ability.
