Overview

We are seeking a skilled Long Term Care MDS Coordinator to join our team at the State of South Carolina Department of Mental Health. As a vital member of our team, you will play a crucial role in supporting the recovery of individuals with mental illnesses by providing high-quality care and services.

Salary

The estimated annual salary for this position is between $56,210.00 and $104,000.00, depending on qualifications and experience.

Job Description

In this dynamic role, you will be responsible for scheduling required Minimum Data Set (MDS) assessments, completing assigned sections of the MDS, and performing nursing assessments, monthly summaries, and other related duties. You will also facilitate Resident Care Conferences, serve as a backup to unit RNs, and assist with monitoring resident falls and weight loss.
