The Nurse Care Coordinator will be responsible for providing high-quality nursing care to our residents, working closely with physicians and other healthcare professionals to implement treatment plans and evaluate patient outcomes. Key responsibilities include:
- Assessing resident needs and developing individualized care plans
- Coordinating with interdisciplinary teams to ensure seamless care delivery
- Administering medications, performing treatments, and documenting patient information
- Monitoring resident progress and reporting any changes or concerns to the healthcare team

- A valid LPN license and current registration
