We are seeking an Admissions Assessment Nurse to join our team at Southview Acres Health Care Center. As a Registered Nurse, you will play a crucial role in delivering high-quality patient care services and increasing satisfaction by facilitating and managing all aspects of the nursing admission process.


Job Summary:
  • The RN Admissions Nurse will assess patients/residents upon admission for appropriateness of admission, coverage, and level of care determination.
  • They will communicate with internal staff regarding coverage and contract details.
  • Proactive work with the multidisciplinary team to manage care, costs, and discharge planning.

Requirements:
  • Registered nurse with current licensure from the State Board of Nursing in relevant states.
  • Bachelor's degree with previous admissions/or intake experience preferred; long-term care, acute care, or subacute experience also preferred.
  • Knowledge of MDS, payer systems, and contracts (Medicare, HMOs, Managed Care, Third-Party Payer Systems).
  • Demonstrated organizational skills and ability to handle multiple priorities accurately and efficiently to facilitate customer and program needs.
  • Experience in human relations, sensitivity to individuals in crisis, good listening skills, and ability to handle inquiries and relate well with referral sources, prospective patients/residents, and families.
  • Ability to maintain confidentiality and handle sensitive information and admission records.
  • Availability to work 11:00 am - 7:00 pm, every other weekend.
  • Fluency in reading, writing, speaking, and understanding the English language to communicate effectively with all customers.

Compensation:

We offer a competitive salary range of $70,000 - $90,000 per year, depending on experience.
