The role involves delivering primary care to high-risk patients, often managing multiple health conditions, within their homes.

The practitioner will lead a multidisciplinary team, collaborating with nurse coordinators, social workers, and other professionals to optimize patient care.

Flexibility is essential, as the role may require coverage across different regions, along with reliable transportation. Minimal on-call duties may also be expected.

Qualifications:

Minimum requirements include:

  • Current state license as an Advanced Practice Registered Nurse (APRN).
  • Certification as a Nurse Practitioner.
  • Eligibility for state and federal controlled substance licenses.
  • BLS Certification for Healthcare Providers.

Preferred qualifications include a Master's degree or higher and relevant certifications from recognized organizations.
