Mental Health Support Specialist

Telecare Corporation is seeking a skilled Case Manager I to serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
Key Responsibilities:
- Assist clients to live as independently as possible in their chosen community.
- Develop and implement individualized service plans to meet the unique needs of each client.
- Provide education and support to clients and their families to promote recovery and wellness.
- Collaborate with community resources and healthcare providers to ensure comprehensive care.
Requirements:
- High School Graduation or G.D. equivalent and two (2) years of direct services in Mental Health or community services.
- Bachelor's degree in Social Services or related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
