Licensed Behavioral Health Case Manager

As a Behavioral Health Case Manager with Kaiser Permanente, you will play a critical role in providing integrated and coordinated care to high-risk and high-utilizing populations in both inpatient and outpatient psychiatry and addiction medicine settings.
Key Responsibilities- Engage with high-risk, high-utilizing patient populations and act as a central point of contact for referrals from multiple departments.
- Provide assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities, and networking skills.
- Plan services based on assessment of both short and long-term needs and track referrals and outcomes for ongoing quality management.
- Facilitate collaboration between Primary Care, Behavioral Health Care Services, and Healthcare Providers, providing referrals and follow-up services as needed.
- Link patients to community services, establish linkages with outside resources, and provide brokerage and advocacy when necessary.
- Provide individual and/or group therapy for chronic, high-utilizing patient populations, as well as individual supportive psychotherapy and family counseling when needed.
- Minimum two years of recent experience in case management and working with community resources.
- At least two years of experience in a mental health setting, with experience working in inpatient and/or outpatient treatment services.
- Masters Degree in Social Work, Psychology, Sociology, or other related field related to behavioral health.
- Licensed Professional Clinical Counselor (California), Licensed Clinical Social Worker (California), or Licensed Marriage and Family Therapist (California).
- Bilingual (Spanish) preferred.
