Patient Services Coordinator LPN Home Health Full Time

About the Role:
The Patient Services Coordinator LPN is a vital member of our team, responsible for ensuring seamless care coordination and continuity for our patients. As a key point of contact, you will communicate effectively with field staff, patients, physicians, and other stakeholders to manage schedules, coordinate care, and resolve any issues that may arise.
Key Responsibilities:
- Manage patient schedules, ensuring timely reassignment of visits and updates to agent unavailability in the worker console.
- Initiate infection control forms as needed and send completed reports to HRD for upload in the worker console.
- Provide backup support during busy periods, including receiving calls from field staff and assisting with weekly case conferences.
- Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff.
- Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
- Process reassigned and rescheduled visits, ensuring supervisory visits are scheduled.
- Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly Agent Schedules, reviewing for productivity and geographic issues before forwarding to the Branch Director for approval.
- Verify visit paper notes in the scheduling console as needed.
- Assist with internal patient transfers between branch offices.
- Perform lab report assessments, fax copies to doctors, and route to Medical Records Department as necessary.
- May be required to perform patient visits and/or participate in on-call rotation.
Requirements:
- Be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
- Have at least 1 year of home health experience.
- Prior packet review/QI experience preferred.
- Coding certification is preferred.
- Must possess a valid state driver's license and automobile liability insurance.
- Must be currently licensed in the State of employment if applicable.
- Must possess excellent communication skills, ability to interact well with diverse individuals, strong organizational skills, and ability to manage and prioritize multiple assignments.
