Key Responsibilities:

  • Conducting comprehensive interviews with clients to assess their needs.
  • Facilitating discharge planning to ensure smooth transitions for clients.

  • Providing navigation assistance in various settings.
  • Collaborating effectively with community partners and stakeholders.

Work Environment: This role requires presence in clinic locations, field offices, and correctional facilities.

Compensation & Benefits:
  • Hourly pay rate: $28.71
  • Worksite locations include various community and healthcare settings.

Qualifications:
