Job Title: Social Work Case Manager

In the capacity of a Social Work Case Manager, this role is dedicated to enhancing the well-being of patients and their families by addressing intricate psychosocial and financial challenges. Utilizing advanced social work skills, the case manager mobilizes resources to alleviate risks associated with social determinants that affect patients and families. The primary objective is to provide culturally sensitive support, ensuring that care interventions are executed with respect for diverse populations.

  • Identifies patients and families requiring ongoing care coordination or community resources
  • Collaborates with the healthcare team to formulate safe and timely discharge strategies
  • Facilitates post-acute discharges for patients with complex needs in partnership with the Care Management team
  • Conducts family meetings to discuss advance directives, comfort measures, and other care-related decisions
  • Advocates for efficient discharge planning and comprehensive patient care

Minimum Requirements:
Education: Master’s degree in Social Work is mandatory
Experience: Minimum of one year in discharge planning within an acute or subacute environment
Licensure: LMSW/LCSW is preferred; ACMA certification is also preferred

Physical Demands: The role requires standing for extended periods, walking, bending, and lifting. Proficient communication, auditory and visual skills, attention to detail, and the capability to lift/push/pull 20-50 lbs are essential. There may be exposure to chemicals, blood/body fluids, and infectious diseases.
