We are seeking a skilled Travel Nurse RN Case Manager to join our team at OneStaff Medical. As a Case Management Nurse, you will play a critical role in coordinating long-term care for patients, ensuring they receive the best possible care in the most cost-effective way.
Responsibilities:
- Coordinate long-term care for patients, including developing and implementing care plans
- Work with patients, families, and healthcare teams to ensure seamless care transitions
- Research and stay up-to-date on the latest treatment and procedures
- Collaborate with insurance companies to secure necessary authorizations and approvals
- Provide education and support to patients and families on disease management and self-care
Requirements:
- Current RN licensure in the state of Virginia
- Minimum 1 year of experience in case management or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records and other healthcare software
