Job Description

As an SFC Case Manager, you will play a vital role in ensuring that every family we serve receives the assistance they require to navigate the VA Waiver Program and access essential services.

Your Key Responsibilities:
  • Empower Families: Provide critical support to families in need, helping them to achieve self-sufficiency and independence.
  • Home Visits: Conduct regular home visits to review essential documents, gather necessary signatures, and ensure uninterrupted services.
  • Collaboration: Work closely with the Data Clerk to ensure seamless communication and coordination of services.
