Clinical Care Specialist

Mass General Brigham is currently seeking a skilled Clinical Care Specialist to join our team in South Hamilton, Massachusetts.
About the RoleWe are committed to providing high-quality, patient-centered care and are looking for an experienced professional to deliver specialized services in a dynamic environment.
The successful candidate will work as part of an interdisciplinary team to provide comprehensive care to patients in various settings, focusing on assessment, teaching, treatment, documentation, and care coordination. The ideal candidate will possess excellent communication skills, be able to work effectively with diverse populations, and have a strong understanding of electronic documentation systems.
Key Responsibilities- Deliver high-quality clinical services to patients in various settings
- Assess, teach, treat, document, and coordinate care for patients
- Work effectively as part of an interdisciplinary team
- Utilize electronic documentation systems and communicate effectively with healthcare providers
- Graduate of an approved School of Nursing
- Current Massachusetts license as Registered Nurse
- Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred
- Ability to travel within the Mass General Brigham Healthcare at Home geographic area
- Strong interpersonal and customer service skills, with ability to work with diverse populations
- Effective communication skills, both written and verbal
- Ability to work independently and as part of an interdisciplinary team
The estimated salary for this position is $80,000-$110,000 per annum, depending on experience.
