Position Summary: We are seeking a full-time Case Manager to join our team. This role is essential in providing a wide array of services aimed at assisting individuals and families dealing with complex medical conditions, whether physical or mental. The Case Manager will work diligently to identify clients' needs and objectives, collaborating closely with relevant contacts to determine the most effective treatment pathways.
Key Responsibilities:
- Facilitate psycho-educational groups and individual case management sessions as required.
- Conduct comprehensive intake assessments.
- Lead treatment team meetings and family consultations.
- Manage an assigned caseload, prioritizing clients' urgent needs.
- Perform thorough mental or physical health evaluations to create actionable plans, establish goals, and identify necessary resources.
- Provide counseling in both group and individual formats.
- Collaborate with external agencies to enhance support services and resources.
- Maintain detailed patient records to track clients' progress throughout their treatment.
- Develop comprehensive discharge plans tailored to client needs.
- Fulfill additional duties as necessary.
Qualifications Required:
- Understanding of Federal, State, and Municipal regulations governing the treatment of addiction, including confidentiality and patient rights.
- Knowledge of workplace policies regarding drug-free environments and violence prevention.
- Familiarity with addiction issues and the principles of the 12-Step Recovery Program.
- Experience with patient care documentation and histories.
- Proficiency in crisis intervention techniques and emergency procedures related to substance overdose, including First Aid and CPR.
- Awareness of the effects of alcohol and addictive substances on individuals and their families.
- Strong communication, observational, and writing skills.
Preferred Qualifications:
- LADC-2 Certification.
- 3-5 years of experience in case management within a behavioral health context.
Essential Skills:
- Effective problem-solving and stress management abilities.
- Capability to work collaboratively with a multidisciplinary team to support physical health aspects of recovery.
- Commitment to maintaining quality and safety standards.
- Ability to observe, assess, and document symptoms and progress.
- Provision of basic therapeutic care and appropriate interventions for the population served.
- Support for the daily needs of clients with mental, emotional, or co-occurring diagnoses.
Educational Background:
- Bachelor's Degree preferred.
Physical Requirements: The role may involve exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently.
