About the Position

We are seeking a skilled RN to join our team as a Case Manager, providing exceptional patient care and support in Ventura.

Key Responsibilities:

  • Develop comprehensive care plans based on nursing diagnoses to ensure optimal patient outcomes.
  • Conduct thorough initial assessments of patients and their families to determine home care needs.
  • Regularly evaluate and revise patient nursing needs to ensure timely adjustments to care plans.
  • Initiate and implement care plans, making necessary revisions as patient status and needs change.
  • Counsel patients and their families on meeting nursing and related needs, providing guidance on preventive and rehabilitative nursing procedures.
  • Provide health care instructions to patients as appropriate, per assessment and plan.
  • Identify discharge planning needs as part of care plan development and implement prior to discharge.
  • Perform other tasks as required to ensure high-quality patient care.

Requirements:

  • Minimum 2 years of RN experience with a proven track record of success in case management or a related field.
  • Valid driver's license for regular travel to patient locations.
