Job Description

Job Summary:

Soleo Health Inc is seeking a highly skilled and experienced Clinical Nurse Specialist - Infusion Therapy to join our team. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and support to our patients receiving infusion therapy in the comfort of their own homes.

Key Responsibilities:

  • Assess and Evaluate Patients: Conduct thorough assessments and evaluations of patients to identify their unique needs and develop personalized care plans.
  • Provide Education and Support: Educate patients and their caregivers on the safe administration of infusion therapy, provide emotional support, and address any concerns or questions they may have.
  • Coordinate Care: Work closely with our interdisciplinary team to coordinate patient care, including communication with prescribers, pharmacists, and other healthcare professionals.
  • Monitor and Document Progress: Continuously monitor patient progress, document any changes or concerns, and adjust care plans as needed.
  • Collaborate with Team Members: Work collaboratively with our nursing team, including on-call rotations, to ensure seamless patient care and support.

Requirements:

  • Education and Experience: RN degree, BSN preferred; minimum 3 years of nursing experience, with background in infusion therapy.
  • Certifications and Licenses: Current nursing license for applicable state; advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
  • Skills and Abilities: Strong knowledge of infusion therapies, excellent communication and interpersonal skills, ability to work independently and as part of a team, and proficiency with Microsoft Outlook and Microsoft Office Suite.
