About the Role

Altais Health Solutions is seeking a skilled Patient Care Navigator to join our Inpatient Care Management team. This role requires individuals to work remotely, Wednesday through Sunday, 3:00 pm - 11:30 pm. As a Patient Care Navigator, you will be responsible for assessing, planning, implementing, and evaluating the options and services needed to meet the member's health needs across the care continuum.

Key Responsibilities
  • Evaluate requests for authorization, ensuring appropriateness, verifying eligibility, and interpreting benefits and medical necessity.
  • Utilize clinical knowledge, problem-solving, and decision-making skills to evaluate requests and make informed decisions.
  • Communicate effectively with internal and external partners to facilitate timely access to care.
  • Assess and triage patients, providing concise SBAR reports to support decision-making.
Requirements

To succeed in this role, you will need:

  • A Bachelor of Nursing degree and current California unrestricted RN license.
  • Minimum three years of acute clinical experience in a hospital setting.
  • Managed care experience and discharge planning experience in an acute care or skilled nursing facility are desirable.
  • Basic computer skills, including electronic medical records, Outlook email, and Microsoft Office suite.
  • Effective verbal and written communication skills to represent the organization and advocate for patients.
Compensation and Benefits

The estimated annual salary for this role is $83,000-$115,000, based on education and experience. Altais offers a competitive compensation package, robust benefits, and opportunities for professional growth and development.
