The estimated annual salary for this role is $100,000 - $125,000, based on experience. At Kaiser Permanente, we value our employees and strive to provide fair and competitive compensation.

This position plays a vital role in ensuring continuity of care through communication in rounds and written documentation, level of care recommendations, transfer coordination, and obtaining authorizations/approvals as needed for outside services.

The Patient Care Coordinator Case Manager will be responsible for developing, evaluating, and coordinating comprehensive discharge plans in conjunction with patients/families, physicians, nursing, social services, and other healthcare providers and agencies.

Detailed Responsibilities
  1. Coordinates with physicians, staff, and non-Kaiser providers and facilities regarding patient transitions and discharge planning.
  2. Develops plans of care and discharge plans, and makes recommendations for alternative levels of care as required.
  3. Performs daily pre-admission, admission, and concurrent utilization reviews using guidelines, institutional policies/procedures, and other information to determine appropriate levels of care and readiness for discharge.
Qualifications
  • A minimum of two years of experience in utilization review, case management, and discharge planning preferred.
  • Bachelor's degree in nursing or a healthcare-related field preferred.
  • Masters degree in case management preferred.
